Quality is Never an Accident: Really?

Practically quality seems to be an accident in most of the industries!
Still quality is considered as conformance to "technical parameters of a product. But in present context quality has two more dimensions (a) delivery (b) competitive price. Both need continual efforts. Are industries really working in that direction?
